Yes, the fifth wheel needs to have a frame capable of supporting whatever hitch you want to install. This includes the downward force (pin weight) and horizontal force (tow weight) that the hitch is rated for.
Many fifth wheels do NOT have adequate frame in the rear for a 'real' hitch (hitch mount bicycle carriers are about their limit). Trying to beef up the frame may be possible, just be aware of what the shift in balance of the trailer will do to you. And be sure you don't exceed the tow weight of your tow vehicle.
And check the laws. There is at least one state where it is illegal to double tow.
